This is an older UIC Class II well in Kanawha county. It received a new permit to operate in 2016. The well's previous permit expired in October 2013 and the operator submitted an application for a new permit in January 2014. The Office issued Orders in February 2014 for a number of wells in the process of being permitted allowing them to inject under their former permit; this is one of those wells. There was a period when the operator injected waste without a permit when their permit expired in 2003. They reported injection on WR-40s through September 2004 and again in November 2007.

  • Injection without a permit. Injection into 2014 (part of period covered by 2014 Order).
  • Injection without a permit. Permit expired in December 2003. WR40s show injection through July 2004, minimal injection in November 2007.
  • 2009 NOV for no MIT since 1994 and another in 2015 NOV for a past due MIT.
  • The driller utilized no cement in the construction of the well. A 6% bentonite gel is behind the surface casing.
