47-041-03175

This is a production well in Lewis county converted to be a commercial injection well.

Well drilled in 1982. Converted for injection in 1997.
Formation into which waste is injected -- Fifth Sand
Geographic location: latitude 39.055146, longitude -80.581658

Major issue

  • Early wells within the injection well's AOR need to be located and current status determined (such as if they were properly plugged).
  • During the well's life Mechanical Integrity Tests have not been conducted in a timely fashion as required by regulation, within 5 years of the last MIT. MITs were conducted in 1997, 2006 and 2013.
  • The 2017 inspection showed that some equipment had been removed from the site and that the tanks had been poorly maintained.
  • The well had perforated production casing above the packer so it requires a special MIT procedure.

Underground Injection Control Class 2 Wells

Introduction to Class 2 Wells in West Virginia

Collected documents and information for a number of Class 2 disposal wells and secondary recovery area permits

Searchable tables for UIC inspections, UIC permitting and MITs based on a copy of the Office of Oil and Gas' online UIC databases made in 2016.

Background materials related to West Virginia's primacy (including the primacy application), EPA guidance documents, and a lot more.

Select wells which present important issues

A UIC well constructed without any cement behind the steel casing strings to protect groundwater.

A commercial UIC well where activity on the site contaminated groundwater.

A commercial UIC well where activity on the site or because of well failure has contaminated surface water.

A UIC well whose use led to earthquakes in the area.

Gas Well Study is the examination of natural gas wells in West Virginia.
