Major issue

  • The facility is constructed over and next to a former coal mine pump house and related pipes and conduits. The gravel base for the UIC facility is not impervious to spills and leaks and there is a concern for groundwater contamination.
  • In September 2004 the Office sent a Notification of Non-Compliance since the operator had yet to file the required WR-35 well completion report after drilling the well in 2001. The letter promised further enforcement action if a WR-35 wasn’t filed within 15 days. We’ve seen three WR-35s for this well, in with the drilling permit file collection, one filed in 2017 and another in 2018, long after the 2004 Notification was written.
  • The operator received a NOV in November 2016 for filing a UIC 2D permit renewal application 60 days before expiration of a permit; regulation requires filing at least 90 days in advance if interruption in injection is not desired.
