Major issue

  • The permit expired in 2002 and was not renewed until 2004. During the time that there was not a permit the operator reported monthly injection. Injection without a permit is a major violation. None was issued by the agency.
  • The well has been abandoned since the previous permit expired in 2014. State law requires speedy plugging or a special applied for exemption. No exemption was applied for.
