This is a conversion to injection in Clay county. It has the same operator and similar facility layout as 47-079-01452.

 

Well drilled in 1974. Apparently converted for injection in the mid-1990s (a 1997 inspection report noted new tanks and building).

Major issues

  • There is a large time gap between the 1998 and 2008 MITs. A shorter gap existed between the 2008 and 2014 MITs. MITs must be conducted within 5 years.
  • WR-40 reports show injection taking place without a timely MIT between 2004 and 2007. A 2006 UIC inspection report shows the well was injecting at that time but no violation was issued. Injection without an MIT should receive a notice of violation and financial penalty. None was issued for this major violation for years during self-reported violation of law and the Office’s observation of the well during a period of violation.
