We use a form to record specific information about what we see at a well site. It's often helpful to use the back of the sheet to make a sketch of the site and add other information in greater detail. We also find it's helpful to carry along a notebook in case more space is needed.
If soil or water samples are taken, their locations should be indicated on the site sketch along with an indication of location (distance to wellhead, for instance, and compass bearing). The chloride concentrations should be noted on the form or attached sheet.
