This is a well in Kanawha county drilled in 1977 to be a production well and was later converted to be a non-commercial injection well.

Major issue

  • There is a 1986 NOV for the operator using the well as an injection well without a permit.
  • The online permitting database showed a gap between a permit expiring in August 2002 and a new permit being granted in May 2008. Examination of WR-40 reports on the document database showed almost continuous monthly injection during this period with a permit. A shorter period of injection without a permit exists between the expiration of a permit in May 2013 (an application for a new permit was filed in August 2013) and a new permit being granted in December 2013. No violation was issued by the agency..
  • A NOV was issued in 2011 after an agency inspection found pressure on the backside of the production casing showing a failure of mechanical integrity. A notice of abatement was issued after well repairs and a successful MIT.
