Major issue

  • For a short period between the expiration of a UIC permit in August 2008 and a new permit being granted in December 2008 the operator injected waste without a permit, a major violation. No violation was issued by the agency.
  • Mechanical Integrity Tests (MITs) are required by permit and regulation to be conducted within 5 years of the previous test. There is a lengthy gap between an expiration of an MIT in 2009 and a new MIT in 2012 where the operator injected waste continually without a permit as indicated by self reporting on monthly WR-40 forms. Injection without a valid MIT is a major violation but no notice of violation was issued by the agency.
