Major issues

  • Well was drilled with three strings of casing, 8 1/4, 6 3/8, and 4 1/2 inch, with only 4 1/2 inch casing left in the well with a minimal amount of cement. Later a string of 7 inch casing was installed in the open hole behind the 4 1/2 string and that was cemented. This is not good construction for a UIC class II well to protect groundwater.
  • Inspections have found the operator injecting over the permitted Maximum Injection Pressure (2004, 2008). No violations were issued.
  • Operator WR-40 reporting show injection without a permit for the period after their permit expired in 2011 until they received a new permit in 2012 (October 2011 until April 2012).
