This is an older UIC Class II well in Roane county. The operator received a new permit to operate in 2016. The operator's permit expired in November 2013 and the operator submitted an application (with deficiencies) for a new permit in January 2014. The Office issued an Order in February 2014 allowing the operator to inject waste under the conditions of the expired permit. The Office issued an Order in January 2015 revoking the 2014 Order and prohibited further injection. The operator reported injection from December 2013 after their permit expired through 2016 until they were granted a permit to legally inject waste.

  • Injection without a permit. Injection into 2014 (part of period covered by 2014 Order), through 2015 and after 2015 Order prohibited further injection before they finally received a new permit in 2016.
