UIC II and III permit application package, including forms and UIC guidelines, used by the Office of Oil and Gas before May 2014. The current application package and guidance. The Office also has a UIC well work permit package. It's worth spending some time going through these to get an idea of what the requirements were prior to 2014 and what they are now.
WR-37 MIT form is based on the WW-3B form in the permit application package and it's easy to confuse the two. Usually there is also a large circular pressure device record which may, or may not, be attached to the WR-37.
WR-40 monthly injection report. State law requires monitoring maximum injection pressure and fluid volume daily. Federal law also requires monitoring of the flow rate. Permits usually require monitoring of annulus pressure.
The Office has begun using a more detailed UIC inspection form (UIC-1R-7 which appears have been used starting in 2017) which replaces the prior Office's UIC inspection form (UIC-IR-1). The UIC inspection focuses on actual pressures/volumes while injecting while an inspector is present and also on normal well inspection criteria, such as proper secondary containment for tanks.
The Office also has other inspection forms which can be found in files. The more detailed well inspection form is less often found. Sometimes inspection reports include photographs and other information.
The Office sometimes issues a fine for a NOV. The assessment is done according to an established policy. We have only seen one assessment for a post-2000 NOV.
